The online functional assessment for the application does not take long. If you are familiar with your patient’s medical history, it will take:
You will need to create an account using your name, AHPRA number, DOB, email address and mobile number, which will be used for multi factor authentication (MFA). After you have created your account, all subsequent log ins will require you to enter your email address, password and MFA token (sent to your mobile). You will not need to re-enter your AHPRA number or DOB for subsequent log ins, as authentication will be done in the background.
You only need to create one account to access the Medical Review Assessing Fitness to Drive form and the Accessible Parking Permits application.

To complete an application your patient or client or their nominated representative must have a mobile number to provide with the application. This is used for application correspondence and an application cannot be processed without one. If your patient does not have a mobile phone number, there is an alternate process you can find here.
